Fill this form for each child you enroll. We will keep it on file. (So if there are no changes, you will have to submit this form only this once.)
Indicate the program/s you sign up for and pay the amount in full using PayPal. (You would be able to pay either via your PayPal account, debit, or credit card.)
Print out, sign, and bring to the first session our waiver form. No child will be admitted without a signed waiver.
